I recently updated a health record journal that I published almost a year ago. It’s called The Personal Medical Appointment Planner: Be Prepared for Doctor Visits and Emergencies using a Health Record Journal.
I have been meaning to get the final corrections done a while ago, but you know how life just has other plans! Without further adieu…

The Story Behind the Book
I’ve worked as a physical therapist for over 25 years. It’s the same every time. My patient tells me about every physical problem they have and how it interferes with their mobility, their life, and their happiness. They are in so much pain that they can’t function. Or how they are dizzy and have been falling…
They spend what seems like a long time telling me their story while I listen intently. I can’t help them with most of their medical issues because I’m not a doctor. So we address the physical therapy things, and I refer them back to the doctor for the long list of other problems.
But then…
My patient has finally had a follow-up with their doctor. I asked what they said about these issues you have been telling me about. My patient says, “Oh, I forgot to ask.”
Sigh…
I shake my head because this happens,
Every. Single. Time…
I get it. It’s so hard to remember everything. And doctors are limited in the time they can spend with you.
Keeping all your information, questions, and priorities at your fingertips will help you take charge of your own health. Don’t waste precious minutes with your doctor.
Prepare for your doctor’s appointments.
Show up to appointments with this book and a pen. Record your questions ahead of the appointment and write the answers as you ask the questions. When your family wants to know what the doctor said, you can tell them exactly what they want to know.
This journal comes in different colors so everyone in the family can get one!
This book came about as a necessity. I couldn’t find exactly what I needed. Either they weren’t detailed enough or there wasn’t enough space to write.
My son and I both have chronic health issues. I have binders for both of us, and all our appointments were on scrap papers that had to be filed away and somehow, miraculously, not get lost. With this book, I can now keep it all together!
And up next…
I am working on a similar version of this book for those with PANS/PANDAS! It will have many of the same pages, with some extra writing room. It will also have resources to easily share with your doctors, teachers, and family about PANS/PANDAS.
Stay tuned!!!
